Community Paramedicine is a proactive way to support older adults and other community members to live safely and independently in the home they choose. Care doesn’t have to start with a crisis.

Haliburton County Paramedic Services (HCPS) community paramedics help manage a range of needs—from chronic conditions to mental health and substance use—so problems are addressed early, before they become emergencies.

How we can help

Check vital signs
Complete physical assessment
Perform 12- or 15-lead ECGs
Monitor weight
Monitor blood glucose
Assess fall risk and complete home safety checks
Review medications and support safe use
Provide remote patient monitoring (when appropriate)
Offer vaccinations (influenza and COVID‑19)
Connect you with community services (for example: Meals on Wheels, emergency response buttons, and mental health supports)
