Community Paramedicine is a proactive way to support older adults and other community members to live safely and independently in the home they choose. Care doesn’t have to start with a crisis.
Haliburton County Paramedic Services (HCPS) community paramedics help manage a range of needs—from chronic conditions to mental health and substance use—so problems are addressed early, before they become emergencies.

How we can help
- Check vital signs
- Complete physical assessment
- Perform 12- or 15-lead ECGs
- Monitor weight
- Monitor blood glucose
- Assess fall risk and complete home safety checks
- Review medications and support safe use
- Provide remote patient monitoring (when appropriate)
- Offer vaccinations (influenza and COVID‑19)
- Connect you with community services (for example: Meals on Wheels, emergency response buttons, and mental health supports)
